A $500 Scholarship Award will be presented to both a Male and Female student at the SMW Awards Banquet in January 2008.

General Requirements for Eligibility
(1) An undergraduate or graduate college student between the ages of 18 and 30 as of December 31, 2007
(2) Must have an annual membership in USAT
(3) Participated in a minimum of three USAT Sanctioned events since January 1, 2007 and be ranked in the SMW Region

Please complete all information required in each section before returning the application form to the Scholarship Committee. This form must be received by the Scholarship Committee on or before December 1st

Scholarships shall be awarded in compliance with applicable laws related to non-discrimination on the basis of sex, race, age, creed, color, national or ethnic origin, marital status and handicap. Press releases regarding the scholarship recipients will be sent to appropriate media upon selection of recipients. Include a copy of a recent verification of enrollment and a copy of your 2007 USAT Card.
