Scholarship Program Application

This $500.00 Scholarship Award will be presented at the SMW Awards Banquet in February 2007.

General Requirements for Eligibility
(1) An undergraduate or graduate college student between the ages of 18 and 30 as of December 31, 2006
(2) Must have an annual membership in USAT
(3) Participated in a minimum of three USAT Sanctioned events since January 1, 2006 in the SMW Region.

Complete the appropriate sections of the application form and mail it promptly to:

Applications and supporting documents must be received by the Scholarship Committee on or before December 1st.

SMW Regional Scholarship Before you submit this application, type or print in ink all information requested. If additional space is needed to answer questions, use separate sheets and attach them to this application. Please complete all information required in each section before returning the application form to the Scholarship Committee. This form must be received by the Scholarship Committee on or before December 1st

Scholarships shall be awarded in compliance with applicable laws related to non-discrimination on the basis of sex, race, age, creed, color, national or ethnic origin, marital status and handicap. Press releases regarding the scholarship recipients will be sent to appropriate media upon selection of recipients.
